Patient Information

Many people schedule their "cleaning and checkup" appointment for the first visit. For these visits, we do a thorough exam of the gums and teeth, checking for decay and gum disease. If a routine cleaning is indicated, that can be done at the same visit. If a patient has had recent dental x-rays at another office, it is very helpful for us to receive copies of the x-rays; otherwise, new x-rays will usually be needed.    

If you are having a specific dental problem and want to be seen for that before you schedule a cleaning appointment, just let us know what kind of dental concern you have. We will make sure to schedule your first visit with Dr. Kate so that she can do an evaluation of the problem.

If you have dental insurance, it is helpful to provide your policy information when you make the first appointment so we may research your benefits before your visit.

New Patient Forms

If possible, please fill out the patient forms that can be accessed via the link below and bring them with you to your appointment. If it is not convenient for you to fill out the forms in advance, we request that you arrive 15 minutes prior to your scheduled appointment. If you have not already provided your insurance information to us, please bring it with you to your first visit.

Meeting Your Dental Team 

Our receptionist, Kika, or our office manager, Jaime, will be at the front desk when you arrive at our office. They can both help with any necessary forms. Our hygienist, Suzi, or Dr. Kate's assistant, Roxanne, will start your appointment, and Dr. Kate will see you for examination and answer any questions you may have.

Your Dental Cleaning

Dr. Kate and our hygienist, Suzi, have both been cleaning teeth for many years and can provide a thorough but comfortable but thorough experience.

I felt welcome and comfortable

I had a dental emergency and called 15 minutes before they were getting ready to leave and they stayed to treat me. Dr. Kate was wonderful! I don't have insurance but was in a ton of pain and they didn't charge out the wazoo for the start of my root canal. Dr. Kate noticed how much pain I was in and numbed me a little more so I'd be more comfortable for the rest of the night. I don't remember the hygienist's name but she was also just as nice and comforting. The dentist can cause me a lot of anxiety especially when I'm by myself but I didn't have any issues there.

I felt welcome and comfortable and I felt they were concerned about my pain and were gonna do whatever they could to help me. 

Dr. Kate also said she'd do some of the root canal procedure at no extra cost to me if I go back there to have the surgery (I have to have some bone removed). Needless to say I will be calling this afternoon to set up the next appointment to finish my root canal with Dr. Kate. I also really hope they accept my insurance once I get it in October as I'd love to have them be my family dentist. This dentist was amazing I have no complaints whatsoever.
